How do I refer a patient?

Please fax a referral to the Complex Joint Clinic as per the PDF referral form below. 

All referring information must be completed in full and incomplete referrals will be returned. All referrals must contain a radiology report supporting the referral. All referrals must be related to surgical consultation for hip and/or knee reconstruction. Referral wait times can fluctuate.

Please note that we see the following conditions:

Osteoarthritis, Hip dysplasia, Failed Arthroplasty (revisions), Avascular Necrosis and Hemi-arthroplasty. 

 

We will NOT see patients with:

Soft tissue injuries, meniscal /labral tears, ACL, bunions, shoulders, ankles or spinal injuries

 

Diabetic Patients

Please send the most recent HBGA1C result along with the referral and a comment on your patient’s diabetic control. Please ensure your patient’s diabetes is well controlled at the time of referral or there is a plan in place for optimization. Surgery may be delayed if HBGA1C is above 7.

My patient has many questions regarding the pre- and post-surgical process. Where can I send them?
